Use this form to sign up for dog training. All dog owners will receive a request
confirmation e-mail. An Owner/Pet Information form and a Liability Release will
be attached to this e-mail. It is required to send the completed Owner/Pet Information
form back to us via e-mail or FAX (details are in the e-mail) and bring the signed
liability release form to the first class. We will notify you by email
once the starting date has been set. Cost $75 and is payable on the first class date.
